Why is the program needed? How does this program relate to dropout prevention? How will this program assist the participants in achieving success? What will this program accomplish? How will the students benefit from participating in this program?



Clearly describe your project idea and provide examples of envisioned student activities.



Please provide a list of intended activities by month to show that the project is well-planned, and that the intended activities are specific and directly related to the project objective.



Please list two or more specific objectives of the program and, for each, describe the method(s) that you will use to determine whether or not the program successfully met the objective.



In column format, provide specific information on the materials to be purchased with grant funds. You should have four columns - 1) the first column should describe each item, 2) the second column should list where you intend to buy the item, 3) the third column should indicate how many of each item you intend to purchase, and 4) the last column should show the cost. Please calculate the grand total (which should equal the amount requested). Make sure you list all items included in your project description. As a footnote, you should explain any items you refer to that are already available in your school or that you will receive from other sources and therefore don't need to purchase.
